You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or via the Gov.UK website
If you are unable to use the PATCHS online consultation tool to request a repeat sicknote then please contact the surgery via phone.
To start a new sicknote you will need to have a consultation with a clinician so that they can assess your clinician needs and provide a relevantly dated sicknote.
